  • Weight Management Program(WMP)

  • Eligibility Criteria

    The Weight Management Pilot Program is open to 2,800 enrollees, spouses, and dependents who meet the following criteria:

    • BMI of at least 30 or a BMI 27-29 and at least one weight-related comorbid condition (e.g., hypertension, high cholesterol, obstructive sleep apnea). 
    • Age 18 and over.
    • Enrolled in one of the following State Group Insurance Plans: Aetna, Capital Health Plan, Florida Blue or United Healthcare in 2025 and 2026.
    • Referred by an in-network licensed provider during the 2025 plan year.
    • All participants in the 2025 WMP must have met all 2025 program requirements, including wellness program enrollment/engagement and submitting the required year-end report through the online link by 10/31/2025.

    Member Responsibilities

    • Each 2026 WMP participant must:
    • Be supervised by an in network licensed provider during the 2026 plan year.
    • Consent to providing personal and medical information to the Department.
    • Enroll in a plan-approved wellness program available to WMP members through their health plan by the deadline provided in the WMP approval email.
    • Meet the engagement requirements of the wellness program throughout the 2026 plan year.
    • Submit updated health information on the year-end report by October 31, 2026.

    Participants who do not meet the above member responsibilities will be disenrolled from the program.

    Exclusionary Criteria

    • The following excludes enrollees, spouses, dependents from the program:
    • BMI less than 27 or a BMI of 27-29 without a comorbid condition.
    • Under the age of 18.
    • In COBRA.
    • Enrolled in a Medicare Advantage and Prescription Drug (MA-PD) Plan.
    • Women who are pregnant, plan to become pregnant, or are nursing.
    • Did not meet the 2025 WMP requirements, including enrolling in the wellness program and submitting the required year-end report through the online link by 10/31/2025.

Please note that any data shared will not personally identify you or other participants.   • Members of the 2026 WMP will be required to:

    • Enroll in one of the plan-approved wellness programs available to WMP members by the deadline listed in the WMP approval email.
      • The wellness programs vary by health plan. Florida Blue, Aetna, and Capital Health Plan offer wellness programs through Solera. Solera offers seven different wellness programs, and they work with members to find the program that best matches their preferences and goals. Programs include betr, ciba health, habitnu, VP transform, Weight Watchers, wondr, and digbi. United Healthcare offers one plan for all WMP members, Real Appeal.
    • Meet engagement requirements of the plan-approved wellness plan.
      • Each wellness program has engagement requirements to maintain membership. These may include weighing in on a digital scale connected to the app once a week, tracking a minimum number of meals in the app each week, reviewing videos, attending virtual meetings, etc.
    • Submit updated health information by October 31, 2026.

    Members who do not meet the above requirements will be removed from the WMP and access to weight management medications will end.
